The Clinical Pharmacy Specialist provides clinical pharmacy services including delegated medication and laboratory ordering assesment, and consultative services for inpatients and outpatients as part of the healthcare team working under the general direction of the attending physician and the Director, Clinical Pharmacy Services and the Chief Pharmaceutical Officer. The Clinical Pharmacy Specialist serves as the primary interface between the Pharmaceutical Department's service and research programs and the provision of patient care necessary to meet institutional goals.? ? ? ??Minimum Education:?? ? ?
Graduate of a College of Pharmacy accredited by the American Council on Pharmaceutical Education with a Doctor of Pharmacy degree.? ? ? ??Minimum Experience:?? ? ?
Completion of a clinical pharmacy residency or fellowship in Pediatrics and/or Oncology.?
In absence of residency/fellowship training, at least five (5) years clinical experience in oncology or related setting.?
Completion of PGY-1 and PGY-2 residency training preferred. Board of Pharmacy Specialties certification in a relevant specialty strongly encouraged.? ? ? ??Required Credentials:?? ? ?
